The Deduction of Per Capita and Defense Fund for Make-up Dues Form, MLO-80, is used by the Local to report collection of make-up dues to the International.
The International will calculate the amount of per capita and defense fund owed using the base wage provided and deduct that amount from the Local's dues remittance check. NO MONEY SHOULD ACCOMPANY THIS FORM.
Individuals who pay cash dues for an extended period of time should be placed on automatic cash dues by submitting a Request for Recurring Deduction of Per Capita and Defense Fund Form, MLO-81.
